Job Description
The Licensed Social Worker (LICSW) is responsible for providing individual and group counseling, case management services, and collaborating with outside community agencies as needed (DCF, Probation, inpatient detox facilities, etc.). He/she will counsel patients, monitor patients’ progress, while being compassionate and sensitive to their needs and concerns. LICSW will also work with patients’ primary physicians and/ or social workers to continue recovery after treatment on needed basis.
Primary Job Responsibilities:

Monitor patient’s prescription drug and illicit drug usage.

Coordinate inpatient detoxification services if a higher level of care is needed.

Provide the patient with outside community resources and agencies based on their specific needs.

Remain in consistent collaboration with the psychiatric, Suboxone, and medical providers.

Remain in consistent collaboration with the administrative staff and management.

Help patient through conversations, interviews, and observations.

Provide monitoring and continuity of care between visits according to treatment plan.

Set patient’s goals and discuss treatment plans.

Document patient records and progress.

Counsel patient and listen to their reactions.

Provide patient with emotional tools and methods for recovery.

Educate patient, patient’s family, significant others, and relevant parties of patient’s progress, treatment, and future goals.

Collaborate with multidisciplinary team members by making appropriate referrals.

Perform other duties as assigned.
